Pre-Authorized Debit (PAD) Agreement

You authorize Square One Insurance Services (Square One) and your financial institution to debit payments from your bank account for charges arising from your car insurance policy. These debits may be for monthly variable recurring payments and/or one-time payments associated with this policy.

You waive your right to receive pre-notification of the amount of each future debit. However, you understand that regular debits, which may vary from time to time, will occur on a monthly basis.

This PAD Agreement remains in effect until you provide written notice of change or cancellation to Square One at the address provided below. This notice must be provided at least ten (10) business days before the next regularly scheduled debit. A request to cancel this PAD Agreement is not a request to cancel your car insurance policy.

You have certain recourse rights if any debit does not comply with this PAD agreement. For example, you have the right to receive reimbursement for any debit that is not authorized or is not consistent with this PAD agreement.
